Well, not all classes 'should' try to get into melee. Devastators and Havocs should avoid getting that close to enemies (and that’s why they should always have an escort), and while Tacticals have the melee advantage against heavies they should try to engage melee specialists at range.
The biggest issue we see is that we want Tacticals to feel like once they’re engaged in melee, their best chance of survival is to pull out the pistol & knife rather than continue to fire away with their bolter.

How about a short time period (1-2 seconds) in which the knife has to be drawn for melee, or put away for switching to bolter.
I agree that Havocs and Devestators should be almost helpless in melee, but I think the bayonett the heavy bolter of the havocs should in fact increase the melee damage radically. It should be equipable for the heavy bolter. I'd suggest the same damage like a knife for 150-200 lp, it would still be inferior to every other class because the attacks are very slow already
